Common incidence of catheter-related venous thrombosis between midline catheter and peripherally inserted central catheter:a Meta-analysis

Objective To compare the incidence of catheter-related venous thrombosis between midline catheters and peripherally inserted central catheters. Methods Randomized controlled trials and cohort studies on the incidence of venous thrombosis associated with midline catheters and PICC catheters were searched from CNKI,Wanfang database,VIP database,Web of Science,PubMed,Embase and Cochrane Library from inception to December 31,2022. Review Manager 5.4 software and Stata 14.0 software were used to analyse and describe the outcome indicators. Results A total of 16 studies were included,including 12 cohort studies and 4 randomized controlled studies,with 21853 subjects. The results of the Meta-analysis showed that the incidence of catheter-related venous thrombosis per thousand catheter days of midline catheters was statistically significant compared with PICC[RR=2.74,95%CI(1.21,6.21),P=0.016]. There was no significant difference in the incidence of catheter-related venous thrombosis compared with PICC[RR=0.85,95%CI=(0.70,1.03),P=0.101]. In the subgroups,the incidence of superficial vein thrombosis in the midline catheter was significantly different from that in the PICC[RR=2.36,95%CI=(1.56,3.58),P<0.001]. Conclusion The current evidence shows that the incidence rate of catheter-related venous thrombosis per thousand catheter days and superficial vein thrombosis was higher for midline catheters than PICCs. Therefore,in clinical practice,vascular access devices should be selected reasonably,and the occurrence and development of catheter-related superficial venous thrombosis should be paid attention to,and clinical screening should be effectively carried out on the basis of a full evaluation.

Social isolation in cancer patients:an evolutionary concept analysis

Objective To clarify the concept of the social isolation with cancer patients. Methods We searched studies on social isolation with cancer patients from websites and databases including Wanfang,CNKI,CBM,VIP,Pubmed,Web of Science,EBSCO,Embase,Proquest,Springer Link,Elsevier Science Direct,Wiley Online Library,Scopus. We selected papers according to inclusion criteria. Rodgers evolutionary method of concept analysis was used. Results A total of 103 articles were retrieved,and 5 attributes were identified,including the lack of contacts,self-isolation,loneliness,alienation and meaninglessness. Antecedents included demographic factors,factors of disease and treatment factors,psychological and sociological factors. Consequences included the increased bad behavior and decreased physical functioning. Conclusion The concept attributes of social isolation of cancer patients were identified by concept analysis method. In the future,researchers can carry out more comprehensive and in-depth research,combining the attributes of social isolation with cancer patients.

Effect of lavender aromatic therapy combined with emotional freedom techniques on insomnia cancer breast patients with heart and spleen deficiency

Objective To investigate the clinical nursing effect of lavender aromatherapy combined with emotional freedom techniques(EFT) on insomnia patients with breast cancer and heart-spleen deficiency.Methods A total of 140 patients with insomnia due to heart-spleen deficiency of breast cancer who met the inclusion criteria in the department of mammary gland of a tertiary hospital in Hangzhou from September 2020 to July 2021 were selected as the research subjects by convenience sampling method. Factorial design with 2 factors and 2 levels was adopted. According to the random number table method,the patients were divided into a control group(n=32),an aromatic group(n=33),an EFT group(n=32) and a combined group(n=32). The control group was given routine care of insomnia;on the basis of routine nursing for insomnia,the aromatic group was given lavender aromatherapy,and the EFT group was given EFT. The combined group was given the above 3 methods of comprehensive interventions at the same time,and the intervention period was 2 weeks. Pittsburgh sleep quality index(PSQI) was used to evaluate sleep quality within 24 hours and 2 weeks after intervention. Hospital anxiety and depression scale(HADS) was used to evaluate the improvement of negative emotions. The improvement of symptoms was evaluated by traditional Chinese Medicine(TCM) symptom score standard of deficiency of heart and spleen.Results Before and after the intervention,except the control group,the PSQI score,HASD score and TCM symptom score of the 3 groups were compared,and the differences were statistically significant(P<0.05). After intervention,the differences in PQSI score,HADS score and symptom score among the 4 groups were statistically significant(F=10.049,P<0.001;F=9.328,P<0.001;F=14.934,P<0.001;F=3.741,P<0.05),and the combined intervention of the 2 therapies was superior to any single therapy in the overall efficacy. The combined therapy has synergistic interaction effect on improving TCM symptoms of deficiency of both heart and spleen.Conclusion The combination of 2 therapies can effectively improve the sleep quality of breast cancer patients with insomnia of deficiency of both heart and spleen,and it has a magical effect in alleviating bad mood and relieving TCM symptoms.
